As we continue our health and safety education efforts at Warren County Public Schools, we will be conducting an anonymous student survey for WCPS students in grades 6–12 regarding school climate, including bullying, violence, alcohol, and drug use. WCPS and the Warren Coalition will use this data to support ongoing health and safety initiatives across both our schools and the broader community.
Participation is voluntary, and families may opt out by returning the signed form prior to administration, while students may also decline to participate at the time of survey without penalty. To ensure full transparency, here is the opt-out form and the survey questionnaire for your review.
